When Exactly Does It Shake? — This Narrows It Fast

The timing and conditions of the shake are the most important diagnostic clue you have before touching anything. Read through these patterns and match your situation — each one points to a specific cause group.

Shake Pattern → Most Likely Cause

Be as specific as possible — when it shakes tells you more than how much it shakes.

Only when cold, clears when warm
Misfiring cylinder on cold start. Most common cause: one spark plug or ignition coil that works when hot but fails when cold. Also consider a fuel injector that doesn’t open fully until fuel rail pressure builds. On high-mileage engines with 80k+ miles, cold-start misfires that clear when warm are almost always ignition. Check freeze frame RPM — if the code was set below 1,200 RPM during warm-up, that confirms it.
Constant shake at idle, clears above 1,000 RPM
Vacuum leak or EVAP purge valve stuck open. This is the classic vacuum leak pattern — the leak pulls the most unmetered air at low RPM when manifold vacuum is highest. At cruise, the larger airflow overwhelms the leak’s effect and idle stabilises. Check LTFT — if it’s above +8% at idle and drops at cruise, you have a vacuum leak. The carb cleaner test finds most of these in 10 minutes.
Constant shake, same hot or cold
Worn spark plugs, failing coil, or low compression. When the engine shakes identically regardless of temperature, the cause is not temperature-dependent — the component is failing in all conditions. If plugs are over 60k–80k miles, replace the set first. If shaking continues after fresh plugs, do a coil swap test to find a weak coil. Low compression from a worn engine won’t show on a coil swap — needs a compression test.
Shake + stumbling at idle, then stalls
Dirty throttle body, faulty IAC valve, or large vacuum leak. The engine is fighting to maintain idle speed and losing. The throttle body controls airflow at idle — carbon buildup narrows the air bypass passage and the ECU can’t compensate. Try cleaning the throttle body first. If that doesn’t resolve it, check for a large vacuum leak (brake booster or intake manifold gasket).
Shake in gear (Drive) only, smooth in Park/Neutral
Broken or worn engine/transmission mounts. Motor mounts isolate the engine’s vibration from the chassis. When the engine is under load (in Drive with the brake applied), a failed mount allows the engine to rock visibly. This is a purely mechanical issue — no engine codes, normal fuel trims. Rock the engine with your hand in Park — if it moves more than 1–2 cm it’s a mount failure.
Shake after filling up with fuel
EVAP system overloaded or contaminated fuel. Filling up with a near-empty tank can flood the EVAP charcoal canister with liquid fuel. This causes the purge valve to flood the intake with rich vapour on the next start, causing a rich stumble. If the shaking resolves within 10–15 minutes of driving, this is the likely cause. If it persists, consider fuel contamination — wrong fuel grade, water in fuel, or ethanol sensitivity on older vehicles.

Coolant entering combustion — head gasket or cracked head. White exhaust smoke on a warm engine (not just cold-start condensation) combined with rough idle is a serious warning sign. Coolant in the combustion chamber misfires that cylinder and produces steam. Check coolant level — if it’s dropping with no external leak, you have an internal leak. A block test (combustion gas test on the coolant) confirms this without head removal.

Worn or Fouled Spark Plugs
Very Common
Spark plugs are the single most common cause of rough idle and engine shaking. As the electrode wears and the gap widens, the ignition system has to work harder to fire the plug reliably. At idle — where the ignition system is under the least electrical stress — worn plugs are most likely to misfire. A plug that fires fine at 3,000 RPM may miss repeatedly at 700 RPM idle. Plugs with more than 60,000–80,000 miles (30,000–40,000 on copper) should be replaced as the first step in any rough idle diagnosis. Check for oil fouling (black oily deposits) or carbon fouling (dry black deposits) which indicate additional problems — oil burning or rich running — that need addressing separately.

Failing Ignition Coil
Very Common
On modern coil-on-plug (COP) engines, a failing coil produces a cylinder-specific misfire that causes noticeable shaking at idle. Unlike worn plugs which cause gradual degradation, a failing coil tends to produce sudden rough idle that may vary with temperature — coils fail more often when cold or wet. The coil swap test is definitive: swap the coil from the shaking-linked cylinder to a known-good cylinder. Very Common
A vacuum leak introduces unmetered air into the intake manifold. At idle — where manifold vacuum is highest — this pulls the most air and causes the greatest lean condition. The engine hunts for a stable idle as the PCM scrambles to compensate. The classic symptom is rough idle that clears when RPM is raised above 1,000–1,500. Long-term fuel trim (LTFT) above +8% at idle confirms a lean condition. Dirty Throttle Body / Carbon Buildup
Common
The throttle body controls the amount of air entering the engine at idle. Carbon deposits accumulate around the throttle plate and idle air bypass passages over time, narrowing them and making idle airflow erratic. The ECU struggles to maintain a stable idle speed — the engine may hunt (RPM rising and falling), stumble, or feel rough at idle specifically. Direct injection engines (GDI) are particularly prone to this because fuel doesn’t wash the back of the intake valves and throttle body the way port-injected engines do. Failed or Failing Fuel Injector
Common
A clogged or leaking injector causes a specific cylinder to receive either too little fuel (lean misfire) or too much fuel (rich, oily plug, hydrolock risk if severe). At idle, where injection pulsewidths are shortest, a partially clogged injector may not deliver enough fuel to fire the cylinder reliably. The result is a rough idle that often has a regular rhythm — one miss every few seconds — rather than the random pattern of a vacuum leak. A leaking injector can also cause a hard start after sitting, as fuel drips into the cylinder when the engine is off.

Common
Motor mounts are rubber-and-metal assemblies that cushion the engine against the chassis. When the rubber deteriorates or the mount cracks, engine vibration that was previously isolated now transfers directly to the cabin. This cause is unique because the engine itself may be running perfectly — the shaking is a mounting issue, not a combustion issue. EVAP Purge Valve Stuck Open

The EVAP purge valve controls when fuel vapour from the charcoal canister is drawn into the intake for combustion. At idle, this valve should be closed. If it sticks open, it floods the intake with a rich vapour that destabilises the mixture — causing hunting, stumbling, or rough idle particularly at cold start. This is a frequently overlooked rough idle cause because it doesn’t feel like a classic misfire. The clue is an irregular stumble that changes character as the engine warms and purge cycling begins. Low Compression — Mechanical Engine Problem
Less Common
Low compression in one or more cylinders causes a misfire that no amount of ignition or fuel system repairs will fix. Causes include worn piston rings, burned or bent valves, a blown head gasket, or a cracked cylinder head. This is diagnosed last because it’s the most expensive to repair and the least common cause of rough idle on vehicles under 150,000 miles. Warning signs that point here: rough idle that persists after replacing plugs, coils, and checking for vacuum leaks; oil consumption with blue smoke; coolant loss with white smoke; or a vehicle with known overheating history.

What to Check Right Now — No Scanner Needed

Check engine light — solid or flashing?
This is the most important first observation. Flashing = active misfire, stop driving. Solid = fault detected but not an active emergency. No light at all = the cause is likely mechanical (mounts, throttle body carbon) or the fault hasn’t been detected yet. Note the light behaviour before doing anything else.

Does the shaking change in Drive vs Park/Neutral?
Put the vehicle in Drive with your foot firmly on the brake (in a safe, stationary position). If shaking gets significantly worse in Drive compared to Park, motor mounts are suspect. If the shaking is the same regardless of gear, the cause is engine-related (ignition, fuel, air). This test takes 30 seconds and eliminates or confirms mount failure without any tools.

Listen for a hissing or sucking sound at idle
Open the hood with the engine running. Listen carefully around the intake boot (the rubber hose from the air filter to the throttle body), all vacuum hoses, and the brake booster line. A hissing or high-pitched whistling sound that’s loudest at idle and changes when you rev the engine is almost certainly a vacuum leak. This is one of the cheapest and most common rough idle causes and often audible without any tools at all.

Check the throttle body for carbon
Loosen the clamp and slide back the intake hose at the throttle body. Shine a light inside the bore — you’re looking for a thick black carbon ring around the butterfly valve. If you can see the ring, that’s your rough idle cause on many vehicles over 50,000 miles, particularly direct injection engines. Clean it with throttle body cleaner spray and a clean rag. Do not use carb cleaner on MAF sensors or coated throttle bodies.

Check spark plug condition (pull one)
Remove one plug — whichever is most accessible — and inspect it. A healthy plug has a light tan/grey electrode with minimal deposits. A worn plug has a visibly eroded or rounded electrode with a gap wider than spec. A fouled plug has heavy black carbon (rich running) or wet oily deposits (oil burning). If one plug looks bad, replace the full set — they’re all the same age.

Check for white exhaust smoke
Stand behind the vehicle on a warm engine (not just cold-start condensation). White steam that persists after 2–3 minutes of warm running suggests coolant burning in the combustion chamber — a head gasket concern. Blue smoke indicates oil burning. Black smoke indicates rich running from fuel issues. No smoke on a warm engine is a positive sign that mechanical failure isn’t the cause.

Common Rough Idle Causes by Vehicle Make

The same symptom has different most-likely causes depending on the vehicle. These are the highest-frequency causes confirmed on each platform.

Toyota (Camry, Corolla, RAV4)
2AZ-FE engines: cracked intake air hose is the #1 cause of rough idle. Also check PCV hose at the valve cover. Camry 2AZ-FE cold-start shake: often a single cylinder coil failing in cold weather — warm it up and the shake usually clears. Code: P0301–P0304.
Honda (Civic, Accord, CR-V)
K-series engines: IACV (idle air control valve) and throttle body carbon are the top causes. Honda rough idle that clears above 1,000 RPM almost always responds to throttle body cleaning first. VTEC solenoid gasket leak causes lean codes alongside shake on K-series. Code: P0507 · P0171.
Chevy/GM (Silverado, Equinox)

It depends on severity. If the check engine light is flashing, stop driving — an active misfire is damaging the catalytic converter with every minute you drive. If the CEL is solid and the shake is mild, short trips to a shop are usually acceptable. If the shake is severe enough that the engine sounds like it might stall, or there’s unusual smoke or smell, park it and diagnose first. A shaking car that is also overheating needs to stop immediately.
This is the classic vacuum leak or dirty throttle body pattern. Both of these issues have the greatest effect at idle where manifold vacuum is highest and airflow is lowest — the lean or unstable mixture affects idle stability but becomes proportionally less significant at higher RPM and larger airflow. Almost certainly a failed engine or transmission mount. Rarely as a primary cause. A failing upstream O2 sensor causes incorrect fuel trim commands and can contribute to a rough mixture, but it typically doesn’t cause severe shaking on its own. More often, the shaking has an ignition or air/fuel delivery root cause, and a bad O2 sensor compounds it. Always fix vacuum leaks, clean the MAF, and check plugs and coils before replacing O2 sensors — the O2 sensor is far more often a symptom than a cause.
