When Exactly Does It Hesitate? — Narrows the Cause Fast

The conditions under which the hesitation occurs are more diagnostic than the hesitation itself. Match your situation and you’ve already cut diagnosis in half.

Hesitation Pattern → Most Likely Cause

Be as specific as possible about when the hesitation occurs.

Dead stop only — fine once moving
TPS (throttle position sensor) or dirty throttle body. The PCM needs accurate TPS data to calculate fuel enrichment for initial throttle opening from idle. A worn TPS with a dead spot delivers a glitched signal at that exact moment — causing a brief lean stumble. A dirty throttle body creates turbulent airflow at small throttle openings with the same result. Carbon cleaning and TPS testing are the first steps.
Light throttle only — fine at wide-open
Dirty or failing MAF sensor. The MAF has highest accuracy requirements at partial throttle openings where small air mass changes matter most. A contaminated MAF delivers inaccurate readings at partial throttle. At wide-open throttle the PCM uses a backup enrichment strategy that bypasses MAF accuracy — so hesitation at light throttle but not full throttle is the telltale MAF pattern. Clean the MAF first — $8, 10 minutes.
Highway speed / high load only
Weak fuel pump or fuel pressure problem. At idle, fuel demand is low enough that a weakening pump can still deliver. At highway speed under significant load, the pump can’t sustain the required pressure — a lean condition results. If hesitation is exclusively a high-RPM, high-load phenomenon, test fuel pressure under a snap throttle. Pressure should hold within spec — a drop of 10+ PSI under load points directly to the pump.
Cold start only — clears when warm
Coolant temperature sensor or cold-start enrichment fault. The PCM uses the ECT sensor to calculate extra fuel enrichment during warm-up. A faulty ECT sensor reporting an incorrect temperature causes under-enrichment (lean stumble) or over-enrichment (rich bog) on cold acceleration. Also consider a sticking IAC valve that doesn’t fully open when cold.
All conditions — gradually getting worse
Worn spark plugs or degrading ignition coils. Gradual worsening over weeks is the classic ignition wear pattern. As plug gaps widen, acceleration demands the most from the ignition system and is where degraded plugs fail first. If the vehicle has over 60,000–80,000 miles on the original plugs, this is the highest-probability and cheapest first step.
After rain or car wash
Moisture in ignition coil boots. Water entering a cracked coil boot causes a temporary short that misfires under acceleration load. Resolves as moisture evaporates — typically 10–30 minutes of driving. If hesitation reliably follows wet weather, inspect all coil boots for cracks and check for water in plug wells.
Jerky / lurching at 35–55mph
Torque converter clutch (TCC) or transmission issue. A rhythmic mechanical engagement/disengagement feel at light throttle between 35–55mph is often the torque converter clutch cycling incorrectly — a transmission fault, not an engine fault. Look for P0740 or P0741. Low or degraded transmission fluid also causes TCC shudder.

Dirty or Failing MAF Sensor
Very Common
The MAF sensor measures the amount of air entering the engine and is the primary input the PCM uses to calculate fuel delivery. When the sensing wire becomes contaminated with oil vapour or dust, it under-reports actual airflow — causing the PCM to deliver too little fuel during acceleration. This is especially pronounced at partial throttle where MAF accuracy matters most. Cleaning the MAF with dedicated MAF cleaner spray resolves a significant percentage of hesitation cases for under $10 and takes 10 minutes. Try this before any other diagnosis. Worn Spark Plugs
Very Common
Worn spark plugs require more voltage to fire reliably. Under acceleration — where cylinder pressures are highest — a plug that fires fine at idle may misfire during the brief period of maximum demand. The result is a hesitation or stumble under acceleration that is often absent at idle. If the vehicle has more than 60,000–80,000 miles on the original plugs, spark plugs are the highest-probability first repair. Replace the full set — they’re all the same age and will all fail at similar intervals.

Faulty Throttle Position Sensor (TPS)
Common
The TPS tells the PCM exactly how far the throttle plate is open — critical for calculating fuel enrichment for tip-in (initial throttle opening). A TPS with a dead spot or worn potentiometer track causes the PCM to receive incorrect data at the exact moment you accelerate from idle. The result is a characteristic stumble or “bog” specifically during initial throttle application. On scanner live data, TPS voltage should sweep smoothly from ~0.5V at idle to ~4.5V at wide-open throttle with no dead spots or voltage spikes. Any flat section or jump in the sweep confirms a faulty TPS.

Vacuum Leak — Lean Condition Under Acceleration
Common
A vacuum leak leans out the air/fuel mixture at idle and under light throttle. When you accelerate, fuel demand increases sharply — a lean engine struggles to respond with full power because the mixture is already lean before the enrichment pulse is added. The hesitation typically feels like the engine “falls flat” briefly before catching. LTFT above +8% at idle alongside hesitation is a strong lean/vacuum leak indicator. This cause is more likely when hesitation is accompanied by rough idle — the vacuum leak affects both simultaneously.

Fuel Delivery Problem — Pump, Filter, or Injectors
Common
Fuel delivery limitations become most apparent under acceleration because engine fuel demand spikes sharply. A weakening fuel pump may maintain adequate idle pressure but drop below spec under sudden high-demand. A clogged fuel filter creates similar pressure restriction. Dirty injectors deliver less fuel per pulse than commanded — causing lean stumbles particularly when the acceleration enrichment pulse fires. Ignition Coil Failing Under Load
Moderate
A coil that delivers adequate spark at idle may fail to produce sufficient voltage under the higher cylinder pressures of acceleration. This produces hesitation or stumble specifically under load — the engine runs fine at idle but stumbles when throttle is applied. The coil swap test is definitive: swap the suspect coil to a different cylinder and see if the stumble follows or stays on the same cylinder. A coil failing under load typically also produces a cylinder-specific misfire code during hard acceleration.

Carbon Buildup on Intake Valves (GDI Engines)
Less Common — GDI specific
Direct injection engines inject fuel directly into the combustion chamber — intake valves never receive a fuel wash. Over time a thick carbon layer builds on the back of the intake valves, restricting airflow and causing hesitation on cold acceleration. Affects most GDI engines after 60,000–80,000 miles — Ford EcoBoost, GM LTG/LFX, Audi/VW TFSI, BMW N20/N54, Hyundai/Kia GDI. Often no codes are set until severe. What to Check Right Now — No Scanner Needed

Clean the MAF sensor first — always
Before anything else, clean the MAF sensor with a can of dedicated MAF cleaner spray ($8). Spray 10–15 bursts onto the sensing wire inside the MAF housing. Let dry 10 minutes, then test drive. This resolves a meaningful percentage of hesitation cases at essentially zero cost. Never use carb cleaner, brake cleaner, or compressed air on MAF wires.

Pull the air filter out of the housing and hold it up to light. If you can’t see light through it, replace it. Remove the most accessible plug and inspect the electrode. A worn plug has a rounded, eroded electrode with a gap wider than spec. A fouled plug has heavy carbon or oily deposits. Listen for a vacuum hiss at idle
With the engine idling and hood open, listen for hissing, sucking, or whistling near the intake area. Trace it by moving your hand near hose connections and the intake boot — you can sometimes feel airflow near a significant leak. A vacuum leak causing rough idle will also cause hesitation since the mixture is lean before you even start accelerating.

Note hot vs cold behaviour
Worse cold, fine when warm: ECT sensor or cold-start enrichment. Same hot and cold: ignition, MAF, or fuel delivery. Cold-only hesitation points to the cold-start enrichment system. The PCM uses the coolant temperature sensor to calculate extra fuel enrichment during warm-up. A faulty ECT sensor causes under-enrichment (lean stumble) or over-enrichment (rich bog) on cold acceleration. Also check the IAC valve — a valve that sticks at cold temperatures causes hesitation until it frees up as the engine warms. These are both inexpensive to test and replace.
Hesitation followed by a surge is a classic TPS dead spot or EVAP purge valve fault. The dead spot on the TPS causes a brief lean event, the PCM overcompensates with enrichment, and the engine surges as the mixture recovers. Not always. A misfire is a specific combustion failure that produces a P030x code and a noticeable shudder. Hesitation is broader — it includes TPS faults, lean conditions, fuel delivery limitations, and throttle body carbon that may not set any codes. Hesitation with a P030x code means a misfire is causing it. Hesitation with no codes means the cause is likely a sensor fault or lean condition that hasn’t crossed the code-setting threshold yet.
